Miscarriage change

My daughter was 11weeks pregnant & on the 6th of January everything was great strong heartbeat & measured perfect & was told today January 27th that from the measurements that she miscarriage on January 8th two days after her ultrasound (vaginal) & there was no heartbeat but has never bleed or seen anything in three weeks a few days ago she noticed just a little yellow discharge but very vaguely,so why is this happening without any signs of a miscarriage??
